Return forms to Royal Hotel, Penrith, between 7:30 & 8:30pm on
Thursday 31st May 2012 using cash or cheque, or by post to the
PDPL General Secretary, 48 Brougham St, Penrith, CA11 9DH.
Entries by post must only be paid by cheque, made payable to the Penrith & District Pool League.
The Summer League competition will be a league format followed by the top 4 teams in each
Division going through to a knockout competition.
LATE ENTRIES MAY NOT BE ACCEPTED

If a player is under 18 before the 31st May 2012 please mark a X next to their name.
You need a minimum of 4 players to play a match. The format will be 4 singles followed by 1 doubles then another 4 singles and another doubles. This means that you could play 2 singles matches each and 1 doubles each. No-one is allowed to play in both doubles match. Or you could have 12 players where there are 8 players for the singles and another 4 different people that play the doubles.
